Heavens Medical was founded by Dr. Jason Heavens to provide a more personalized, preventive, and relationship-centered approach to medicine. With over 25 years of experience in family and emergency care, Dr. Heavens created a space where wellness, longevity, and vitality come first. Through advanced diagnostics, hormone therapies, peptide treatments, and aesthetic care, our Tempe-based practice helps patients achieve optimal health from the inside out. Dr. Heavens also leads our sister clinic, Heavens Urgent Care (heavensurgentcare.com), offering compassionate same-day medical care for life’s unexpected health needs.

Meet Jason Heavens, MD

Dr. Jason Heavens is a board-certified family physician with more than 25 years of experience in family and emergency medicine. A graduate of the University of Ottawa, Canada, in 1997, he completed a combined residency in both specialties and has practiced in diverse rural and urban settings throughout his career. His approach to healthcare is rooted in prevention, longevity, and personalized wellness—helping adults take control of their health and feel their best at every stage of life.

Dr. Heavens focuses on proactive, evidence-based care designed for people who want to live not only longer but better. His MDVIP-affiliated practice in Tempe, Arizona, offers advanced screenings, diagnostics, and customized wellness programs that go far beyond conventional primary care. With a strong focus on hormone optimization, functional medicine, fitness, nutrition, and preventive care, he partners with patients to uncover root causes, improve vitality, and enhance resiliency in daily living.

With specialized experience in longevity and geriatric medicine, Dr. Heavens provides comprehensive care for midlife and older adults—including those interested in hormone balance, metabolic health, and age management.
